Schools will provide parents with the 2021-22 form at least 30 days prior to the start of testing.
Below please find forms for opting out or exempting your student(s) from state assessments. If parents/guardians wish to opt-out or exempt their student(s) from state testing, they must download, print, complete, and sign the form(s) and submit the form(s) to the school that their student(s) attends for the appropriate tests.
Facts to consider
Before submitting a form, please consider these facts:
State assessments are a measure of student academic achievement related to state academic standards.
State testing does not in any way negatively impact students regarding their regular academic achievement and/or work at school.
Results from state assessment help teachers at your student(s) school improve their teaching practices.
Results are used by school administrators to help teachers adjust instruction to better focus on student academic needs.
Students do not “pass” or “fail” these assessments. They are simply a measure of academic achievement at specific grade levels to help inform instructional decision-making.
